• Participant Registration Form
  This form must be completed for all non-exhibiting attendees who are
  staying at the Samoset or who will be commuting.
  Please include your dinner and lunch choices on this form.

• Exhibitor Registration Form:
  All vendors wishing to exhibit must complete this form.

  Updated daily - don't miss out on your opportunity to reserve the booth of your choice.
• Concurrent Afternoon Workshops - Customer Service &
  Technician Workshop Form:
  The form must be completed for all attending workshops.
• Hotel Registration Information: Registration Deadline is April 22, 2013.
  You may call the Samoset directly to reserve your room; remember to tell
  them you are attending the TAM/TANE convention. You may also make your

• Sponsorship Form: Increase your visibility:
  There are many sponsorship opportunities. There are the Platinum, Gold,
  Silver and Bronze general sponsorships, as well as special event
  sponsorships such as the Wednesday night entertainment. Your company
  name will be displayed on signs and in the program. (must have forms in
  by April 22, 2013 to appear on signs and in program.)
• Golf Hole Sponsorship:
  If you sponsor a hole, TAM will proudly display your company name with a
  sign located on the golf course and on the program for only $130.00.

• Golf and Tennis Registration Forms:
  Please fill out form and pay if you want to participate in the golf
  tournament or to play tennis.
• 2013 ITPA Silent Auction:
  Donations requested for the 2013 ITPA Silent Auction to benefit the
  ITPA-NE Chapter Scholarship Fund
